How to File an Online Claim
Flexible Spending Accounts members can now submit claims online and monitor the status of the claim. When you enter the necessary claim information online, an FSA Online Claim Entry Form is created for you to submit with your supporting documentation. To submit a claim online, please follow these steps:
- Login to the FSA Claims application.
- Click on Submit for the appropriate account.
- Enter the Requested Amount, Dependent Name (if applicable), and the Provider's Name.
- Specify how you will be submitting your form and documentation (fax, mail or e-mail).
- Enter your User ID and PIN.
- Click on Submit Signed Claim.
- Print the FSA Online Claim Entry Form. Fax, mail or e-mail this form along with your supporting documentation for reimbursement
